With advertisements for Viagra and the Atkins Diet overwhelming our media, I could not resist reviewing this interesting Italian trial that studied two key objectives of our patients: losing weight and sustaining erectile function. The prevalence of obesity increased dramatically in the United States during the 1990s; nearly one third of adults (30.5%) were classified as obese in the 1999-2000 National Health and Nutrition Examination Survey. Since 1960, the prevalence of obesity in American adults has increased by 17.5%. When rates of “overweight” and “obesity” are combined, the estimated prevalence among American adults is 64.5%.1 A Canadian population survey of people older than 12 (aboriginal people excluded) found that a quarter of all women and a third of all men were obese (body mass index [BMI] 27 or greater).2
Estimates of rates of ED are less precise. The Health Professionals Follow-Up Study revealed ED rates of 12% in men younger than 59, 22% in men 60 to 69, and 30% in men older than 70.3 Epidemiologic evidence links well recognized risk factors for coronary artery disease, such as obesity, hypertension, and hypercholesterolemia, with ED. Some even suggest that ED can be a sentinel event for coronary artery disease in asymptomatic men.4,5

While obesity is perhaps the biggest source of many problems we see in family medicine, no trials show that we know how to help patients achieve sustainable weight loss. American studies have shown that, at any given time, 40% of women and 20% of men are dieting. Most studies show, however, that one third to two thirds of any initial weight loss is regained within a year, and almost all the weight is regained within 5 years.6,7

Weight loss typically comes from both reduced calorie intake and increased physical activity, but it seems to be increased physical activity that could help reduce rates of ED. Men who initiated physical activity in midlife in the Massachusetts Male Aging Study had a 70% reduced ED rate compared with sedentary controls.8 My own opinion is that advertising for products and diets plays a big role: it both helps by increasing awareness and hinders by suggesting that using the product or diet leads to instant fabulous sex or weight loss. This trial asks whether there can be a common solution to two common problems: can lifestyle change effect weight loss and improve ED?

About Erection

What causes an erection?


Hormones, blood vessels, nerves, and muscles must all work together to make an erection. Your brain starts an erection by sending nerve signals to the penis when it senses sexual stimulation. Touching may cause this arousal. Another trigger may be something you see or hear. It may be a sexual thought or dream.

The nerve signals cause the muscles within the penis to relax and let blood flow into the spongy tissue within the penis. Blood collects in this tissue like water filling a sponge. The penis becomes larger and firmer, like an inflated balloon. The veins then get shut off to keep blood from flowing out.

After climax, or after the sexual arousal has passed, the veins open back up and blood flows back into the body.

About Orgasm

What is Orgasm?


Orgasm is considered to be the culminating event. Masters and Johnson (1996) described the male's and female's sexual arousal response cycle in four parts:
  1. the excitement phase,
  2. the plateau phase,
  3. the orgasmic phase, and
  4. the resolution phase (also includes refractory phase for males).
This article focuses on the orgasmic phase; but remember that all the stages directly or indirectly related to each other and finally to orgasm. The first stage, excitement, refers to body's physiological response to psychological/physical sexual stimulation. If the individual maintains the excitement phase, the second, or plateau, phase is entered. Sexual tension increases if the individual continues to find the sexual stimuli exciting. The ultimate release of continuing tension results in orgasm, contraction of the sex organs.

Difference between male and female orgasm


During the orgasmic phase, involuntary climax is reached, with sensations occurring for females primarily in the clitoris, vagina, and uterus. For men, orgasm includes sensations mainly in the penis, prostate, and seminal vesicles. The final stage, resolution, in the inevitable loss of tension. Added to this cycle for men is the refractory period, the time during which the penis once again achieves tumescence.

Causes of Female Sexual Dysfunction - 5

Discussing more about the causes of Female Sexual Dysfunction (FSD)

We talked about the first and the second perspectives about FSD. Now moving ahead, let us discuss about the third perspective, which can also be known as dissatisfaction theory. This theory is neither medical nor psychological.

This theory is of the view that a major percentage of the sexual dysfunction experienced by females is not due to hormonal deficiency or irregular blood blow to the pelvic region. It is in fact a result of less than required stimulation of the genital region. It is sometimes observed that even healthy females who are relatively younger in age too are affected by sexual dysfunction. This fact supports the above mentioned view.

One of the major reasons for the lack of stimulation in women is lack of communication between the partners and subsequently lack of knowledge in men about stimulating a female. The result of this is ultimately lack of arousal, absence of sexual interest, depression and refraining from sexual activity which ultimately results in an unsatisfactory sexual performance and experience.
